Treatment for Esophageal Varices

Learn about world-class treatment options for Esophageal Varices in India.

About Esophageal Varices

Esophageal varices are swollen veins in the lower part of the esophagus, often caused by liver cirrhosis.

Includes

  • Bleeding Esophageal Varices
  • Non-Bleeding Varices (high-risk)
  • Varices due to Liver Cirrhosis
  • Portal Hypertension
  • Recurrent Upper GI Bleeding
  • Gastric Varices
  • Pediatric Portal Hypertension
  • Variceal Bleed with Ascites or Encephalopathy

Top Treatment Options

Endoscopic Variceal Ligation (EVL / Banding)

EVL is a minimally invasive endoscopic procedure that uses elastic bands to tie off swollen veins (varices) in the esophagus, preventing or stopping bleeding caused by portal hypertension (commonly due to liver cirrhosis).

Advantages
  • 1. Quick and effective for controlling acute variceal bleeding.
  • 2. Minimally invasive with no external incisions.
  • 3. Short recovery time and often done as a day-care procedure.
Limitations
  • 1. Multiple sessions may be required for complete eradication.
  • 2. Does not treat underlying liver disease or portal hypertension.
  • 3. Risk of complications like ulcers, rebleeding, or dysphagia.

Transjugular Intrahepatic Portosystemic Shunt (TIPS)

TIPS is a minimally invasive procedure where a channel (shunt) is created inside the liver using a stent to connect the portal vein to the hepatic vein. This reduces portal hypertension and prevents complications like variceal bleeding and ascites in patients with liver cirrhosis.

Advantages
  • 1. Reduces risk of bleeding from esophageal or gastric varices.
  • 2. Minimally invasive, done via catheter through a neck vein (jugular).
  • 3. Improves ascites control when unresponsive to medication.
Limitations
  • 1. Risk of hepatic encephalopathy due to altered blood flow bypassing the liver.
  • 2. Does not cure liver disease, only manages symptoms.
  • 3. Stent may narrow or block, requiring future interventions.

Sclerotherapy (Endoscopic Injection Therapy)

Sclerotherapy is an endoscopic treatment where a special chemical (sclerosant) is injected directly into or around esophageal or gastric varices to cause them to shrink and stop bleeding. It is often used for emergency control of bleeding or where banding is not feasible.

Advantages
  • 1. Immediate bleeding control – effective in emergency bleeding from varices.
  • 2. Non-surgical & fast – performed through an endoscope via the mouth.
  • 3. Can be done repeatedly – useful in chronic variceal bleeding cases.
Limitations
  • 1. Higher recurrence rate compared to band ligation.
  • 2. Risk of ulcers or strictures at the injection site.
  • 3. Temporary solution – doesn’t address the underlying portal hypertension.
